CLSF serves eligible residents throughout San Francisco who primarily reside in undeserved neighborhoods such as Bernal Heights, Outer/Inner Mission, Bayview and the Excelsior District. (94103, 94109, 94110, 94133)

To participate in Home-Delivered Nutrition Services, an individual must meet one of the following criteria:

• An older adult (60+) living in the City and County of San Francisco who is homebound due to illness or disability, or is otherwise isolated.
• A spouse or domestic partner of an older adult enrolled in the program if an assessment by the grantee’s social worker or assessment staff concludes that it is in the best interest of the enrolled older adult.
• An individual with a disability who resides at home with an enrolled older adult, if an assessment by the grantee’s social worker or assessment staff concludes that it is in the best interest of the enrolled older adult.
